We know that you care how information about you is used and safeguarded. Your responses to questionnaires and assessments on this Web site are entirely voluntary and will be used, anonymously, in ongoing research by Dr. Harvey. By visiting our website - www.tomharvey-me.com - and completing questionnaires or assessments, you are accepting the practices described in this Privacy Notice.

What We Collect and How We Use It - Registration
On the welcome / registration page, we ask you to tell us a bit about yourself, including your age, gender, occupation, role and size of your business. We compute statistics based on the aggregate data for our research on how these factors affect Leader Vitality. In your report you will receive comparative data by age and gender only. Throughout the initial data collection process, you remain strictly anonymous. You are not obligated to provide us with any of this data or engage in any part of this assessment - your participation is wholly voluntary, but if you choose to submit the registration form and take the assessment we require that your provide us with this demographic data and answer all of the questions to receive your interpretive report.

After you receive your initial interpretive report, we will give you the opportunity to tell us your e-mail address, which you may offer voluntarily to request to be added to our newsletter mailing list, to receive more descriptive printed information about the Leader Vitality model, or to use our professional services. You are not required to give us an e-mail address that identifies your personal or professional identity, but we hope you provide us an account that you actively monitor. Trust that your email will be held securely and will not be released to any third party. At your request - by contacting us at tom@leadervitality.com - we will immediately remove your address from our mailing list.

We will also present you with the option of providing your phone number to be included in your contact information. No email addresses or phone numbers will be published, sold or disclosed to any other party, and they will be held securely on our site. If you would like Dr. Harvey to make direct contact with you by phone you can make that know by putting a check in the appropriate box by calling or emailing him directly. (See contact information at the bottom of each page in the assessment).

What We Collect and How We Use It - Assessments
We also collect your responses to the Mini Leader Vitality Assessment on this site and store them, along with your scores, on our database server. We will never make use of, report or disclose your individual responses or scores to others. Your identity throughout the process will be strictly protected. For the purpose of using data for publications, lectures, and other means of communicating results, your individual responses and scores will be aggregated and averaged in with all other scores and responses. We will not include your responses in the comparisons we provide on this site until there are at least 10 assessments from people in the same category (age, gender, occupation, role, and company size).

How We Use Cookies
We will not place any cookies in your browser without your consent. When cookies are allowed they will only be used to manage each session you have at the web site. The cookies make sure that you and only you are able to view your scores, and will never contain any personal information about you. When you close your browser, or log off the site, these cookies will be deleted.

Like almost all Web sites, we automatically collect information on all requests for pages from our Web server. We collect the IP address of your Internet connection (which generally does not identify you unless your Internet service provider has assigned you a static IP address), the type of browser you used, what you requested, and what was sent. This helps us understand how people use the Web site and allows us to produce aggregate statistics on usage.

Security of the Data We Collect
We take reasonable technical, administrative, and physical precautions to keep your information secure. For example, we store your responses in a password-protected database, and data you send to us or we send to you is SSL-encrypted.

Changes
We retain the right to change this Privacy Statement. If we do, the revised Privacy Statement will apply only prospectively to future data collected. We will post changes to this page in a prompt manner. This statement is effective as of January 26, 2012.
